Game Changers Consent Form

Thank you for nominating a Game Changer. The Game Changers campaign is a search for 30 amazing individuals who have done incredible things with National Lottery funding over the last 30 years and is co-ordinated by the National Lottery Promotions Unit (NLPU). Individual nominations will be reviewed by our research team who will then select the winners.

We ask that you seek permission from your nominee for the nomination itself, letting them know what personal data you’re going to share with us, and to send them a copy of this privacy notice.

What are we collecting:

As part of the Game Changers campaign, we are collecting your nominations which will include:

The name of your Game Changer nominee

A description of what the person does within their project

A named contact, email address and telephone number for the nominee

It is the nominator’s responsibility to obtain permission from the nominee to nominate them, and if your nominee is aged under 18, we will also need to confirm parental/guardian permission. In this case we will request the name and telephone number of the parent/guardian.

What we will do with the information you provide:

We will use the information you provide to inform the shortlisting process for the Game Changers campaign.

We will share the information you provide with our entry co-ordinators who will use this information for the basis of shortlisting entrants. In addition, we may use the email addresses supplied to encourage entries in subsequent years. Our coordinators will collect data from public sources to inform decisions on shortlisting entrants.

How long will we retain your personal information:

The deadline for entries for the Game Changers campaign is midnight on 17th June 2024 and we envisage sharing your organisation’s details up until this date.

The data you provide us will be used for this year’s Game Changers campaign and will be retained until May 2025 to inform future campaign entries.

What are my rights?

You have the right to withdraw your consent for the Fund to use your personal data at any time.

You have the right to request access to your information.

You have the right to request that your information be rectified or deleted.

You have the right to request that your information be made available to you in a format that allows it to be reused by other organisations.
